To download a form, simply click on the form name below. Depending on how your computer is set up,
you will see one of 2 things happen:
1. You will be prompted to save the file to your local hard disk. Simply select a location and
name for the file and it will be saved there. You may then open the file (using Adobe Acrobat) and print it.
2. You will see Adobe Acrobat open inside your browser window, and the file will be displayed
here. Click on the print button to print the form. On some computers, you will see
a blank window instead of the Acrobat and the form. If this happens, click the Back button on your browser,
the right-click on the form you want. Select "Save Target As" from the popup menu that appears, then specify
the filename and location in the ensuing form. you may then open the file (using Adobe Acrobat) and print it.
